All requests for time-off should be submitted 7 days prior to the requested date, failure to do so may not guarantee your request approval. Requesting day-off in advance will help your employer to plan for your absence, and if you’re taking an unplanned day-off due to an emergency, you should still request the day off as soon as you can. This form must be signed by the employee requesting time-off and notify your employer after request submission. Employees will be notified upon the request status by email.
